Sacroiliac (SI) Joint Pain

Cause:
The SI joint connects the sacrum (base of the spine) to the ilium (pelvic bone). Inflammation, degeneration, or altered mechanics of this joint produce posterior hip and buttock pain, often mistaken for lumbar or true hip pathology. SI joint pain is typically localized between the lower back and buttock.

Symptoms

  • Dull, aching, localized pain over the SI joint
  • Pain worsens with standing or walking, climbing starts, and transitional movements (e.g., getting out of a car)
  • Less likely to cause numbness or tingling compared to nerve-related pain

Treatment

Conservative Management

  • Anti-inflammatory medications
  • Activity modification
  • Physical therapy emphasizing pelvic alignment, stabilization, and core strengthening

Interventional Options

  • SI joint injections with local anesthetic and corticosteroid
  • Useful for both diagnosis and relief of inflammation
