
Cause:
A hip stress fracture occurs from repetitive mechanical loading that causes microscopic damage to the bone. This is most common in runners, endurance athletes, and military personnel. Without adequate rest—or if nutrition, hormone balance, or bone health are compromised—these micro-injuries can progress from a stress reaction to a stress fracture. The femoral neck is the most frequent site for a hip stress fracture and requires careful evaluation because of its proximity to the blood supply of the femoral head.
Symptoms:
Pain deep in the groin or front of the hip, worsened by weight-bearing activity such as walking, running, or climbing stairs. Some patients experience night pain or discomfort even at rest as the injury progresses.
Treatment:
Rest and activity modification are critical to allow bone healing. Patients are typically placed on protected weight bearing (often with crutches) for several weeks.
Nutritional optimization—including adequate calcium and vitamin D—can help support bone recovery.
If a femoral neck stress reaction or fracture fails to heal with conservative treatment, or is located in a high-risk area, screw fixation may be recommended. This procedure stabilizes the bone and helps prevent progression or displacement, which could compromise blood flow to the femoral head.

Important:
Any groin pain that worsens with weight-bearing should be promptly evaluated by an orthopaedic specialist. X-rays are usually the first step, but an MRI may be needed to detect early stress reactions or subtle, non-displaced fractures.
